According to the document of the Anbar Health Department - Public Health Department, which includes the directive of the Honorable Prime Minister on the necessity of the cooperation of the Ministries of Education and Higher Education and Scientific Research with the Ministry of Health and Environment to facilitate the taking of anti-Coronavirus vaccines for educational and teaching staff and with the concern of the esteemed University of Anbar Presidency which is represented by the President of Anbar University Professor Dr. Mushtaq Taleb Saleh Al-Nada: “ Ladies and gentlemen of the University of Anbar (exclusively) who wish to be vaccinated, are kindly requested to provide us with their data by answering the form below accurately.

Note: a special vaccination outlet for the university will be opened in the main university center.
